31 year old black male, currently under conservatorship, patient is homeless with no outside support. When asked why is he hospitalized he said “I’m not sure”. He is cooperative and friendly, but paranoid. I was able to play games with this patient in the main area.
The whole point of the IPA is to analyze and reflect on a conversation you had with a patient where you used therapeutic communication. This assignment allows you to find ways you were able to explore thoughts and feelings a patient had. This is not an assignment where you ask them where they are, what day it is, whether they are attending to ADLs, and any other intake related close-ended questions

The assignment is broken down into three different parts:
1) An intro describing the patient, their basic demographics, and the environment you had your conversation in. For the environment, make sure you list sights, sounds, distractions, etc that were around you when you had this conversation with the patient.
2) The meat of your assignment will be the chart. This chart has the biggest weight for this assignment so please do well in this section. The chart consists of four columns: one is your communication end, the patient’s communication end, a description of the therapeutic communication techniques used and coping/defense mechanisms identified, and your analysis of that piece of the conversation. You will be documenting FIVE interactions you had with ONE patient i.e. any bits of the conversation you had with the patient that stuck out to you where you used a therapeutic communication skill AND identified anything the patient said that signals the use of a defense and coping mechanism
Student column: There are three parts to this column which are what you said verbally to the patient, your nonverbal gestures when saying what you said to the patient and a goal for this specific interaction. The goal should be something related to gathering a piece of information that you need from the patient or stuck out to you based on what they said. For example, if a patient says they were at the hospital because they wanted to kill themselves, your goal can be like “explore thoughts and feelings about them wanting to kill themselves” or something like that.
Patient column: This is where you put the patient’s response to what you said in the student column and the nonverbal gestures they made while saying what they said to you such as shrugging, eye contact, or lack thereof.
Communication techniques column: This column is VERY important. This is where you identify what techniques you used in this interaction, assess whether it was therapeutic or not AND EXPLAIN WHY. Also, identify any defense or coping mechanisms used by the patient based on their response in the interaction. Students often miss points for not documenting defense and coping mechanisms.
VERY IMPORTANT: Make sure you cite your sources in APA format when discussing the therapeutic communication, defense, and coping mechanism used. Make sure you also have a reference page at the end of this assignment. It’s a big part of the rubric to cite in APA and students often miss this!!!
You can use your book, online sources, or ATI to get information from and cite on. I highly recommend using ATI the most. Everything you need for this assignment can be found in chapters 3 and 4 of your ATI book!
Critique and Analysis: This column is where you talk about whether your goal was reached in the student column or not. Whether it was met or not, discuss why it did or did not. Also, make sure you talk about how you felt with each interaction
3) The conclusion is basically a reflection of how you felt, what went right, what could have been done better, etc.
